What are the FMGE rules for MBBS abroad?
As of 28 August 2026, FMGE rules mean: you hold a foreign primary medical qualification that is a recognised licence to practise in the awarding country; you meet NMC’s screening and eligibility conditions (including NEET from May 2018 on the student-abroad route); and you qualify the Foreign Medical Graduate Examination conducted by NBEMS. Passing FMGE is not a licence by itself. It is the screening step NMC and State Medical Councils then use.
NBEMS states that FMGE was introduced through the Screening Test Regulations, 2002. Indian citizens and OCIs with a primary medical qualification awarded outside India who want registration on or after 15 March 2002 must qualify that screening test. NBEMS’s job is limited: conduct the test, publish the result, and hand it to NMC and State Medical Councils.
Who may apply (as NBEMS summarises it):
- Indian citizen or OCI.
- A primary medical qualification confirmed by the concerned Indian Embassy as a recognised qualification for enrolment as a medical practitioner in the awarding country.
- Final-exam result declared on or before the cut-off date for that FMGE session.
- Eligibility Certificate if applicable, plus the provisional pass / degree certificate of the primary qualification.
NBEMS describes FMGE as the Screening Test introduced through the Screening Test Regulations, 2002. Its role is limited to conducting the test, publishing the result, and handing results to NMC and State Medical Councils. Candidates must also meet NMC’s screening, Graduate Medical Education and eligibility rules.

The original 2002 scheme on NMC’s regulations page is not the live paper. Pattern, dates, qualifying marks and attempts for any session are in that session’s Information Bulletin on the NBEMS site. As of August 2026, NBEMS still lists June 2026, December 2025 and June 2025 FMGE sessions — operational proof that FMGE has not been switched off.
Counsellor insight 1: Families ask for FMGE rules as if they were a pass-percentage table. The binding rule is upstream: if the course fails FMGL, the graduate may never become an eligible candidate. Exam coaching cannot repair a split internship or a non-English clinical year.

For students who joined a foreign medical course on or after 18 November 2021, the FMGL Regulations, 2021 are the NMC foreign medical education requirements that decide later Indian registration. No foreign medical graduate shall be granted permanent registration unless they have completed at least fifty-four months of course, a twelve-month internship in the same foreign institution, an English-medium degree, Schedule-I subjects, a local licence at par with citizens of the awarding country, a supervised twelve-month internship in India after applying to the Commission, and NExT or such other mandated test.

How many years of clinical clerkship are mandatory for MBBS abroad?
FMGL does not ask for “years” of clerkship as a slogan. It asks for a minimum twelve-month internship in the same foreign institution where the primary qualification was obtained, with hands-on training in named clinical subjects, in addition to the 54-month course. After that, a further twelve-month supervised internship in India is required for permanent registration. That is two internships — not one year sold twice on a poster.
NMC’s student advisory restates the 54-month / 12-month / English / local-licence bundle and puts the onus of disqualification on the student.

Who FMGL 2021 does not rewrite: graduates who already held the foreign degree, or were already enrolled, before 18 November 2021 stay on the earlier screening regulations unless a later notice says otherwise (Regulation 5).
WDOMS / WHO / WFME listing is not an Indian licence. WDOMS states that listing does not denote recognition, accreditation or endorsement unless expressly said.
NMC’s For Students to Study Abroad page says the Commission does not endorse any list of foreign medical institutions or universities for MBBS or equivalent. “NMC approved university” is search language for FMGL-compliance — not a stamp NMC issued.

Has FMGE been replaced by NExT?
No. As of 28 August 2026, NBEMS is still conducting FMGE. Treat any blog that says “FMGE has been replaced” as stale. The operational table is in the next section.
What is the NExT exam — and has it replaced FMGE?
NExT is the National Exit Test provided in the NMC Act and in the National Exit Test Regulations, 2023. FMGL Regulation 4(d) already names NExT or such other mandated test. As of 28 August 2026, that “other mandated test” is still FMGE: NBEMS is conducting it, and NMC’s FMG registration guidelines.pdf) tell State Medical Councils to follow the present process until further Commission instructions or NExT implementation, whichever is earlier.

Myth: “NExT is already the exam, so FMGE rules no longer matter.”
Reality: NMC’s FMG registration guidelines still run “till NExT or further instructions.” NBEMS still lists FMGE sessions in 2025 and 2026. Plan for FMGE. Track NExT as a future switch, not as a rumour that the screening test has vanished.
NMC’s FMG guidelines also cap FMG internship allocation in Indian medical-college hospitals (published ceiling: an additional 7.5% of permitted seats). That is a slot problem after you pass — not a reason to skip FMGL at admission.
What changed in NMC / FMGE rules in the last 18–24 months?
The 2021 FMGL tests did not disappear. The newest clarification families are typing into Google is UGMEB’s 18 March 2026 public notice on physical onsite compensation for online MBBS study (COVID/war batches) — it withdrew a 6 March 2026 notice. UGMEB also re-alerted joiners in November 2024; April 2026 reporting covered named Uzbekistan campuses. FMGE rules stayed operational through the June 2026 session listed by NBEMS.
What is the NMC March 2026 online-class compensation rule?
It is a clarification for Foreign Medical Graduates whose theory, practical or internship ran online (typically COVID-19 or war disruption), not a rewrite of FMGL for 2026 joiners. UGMEB notice No. U-15024/15/2024-UGMEB(Pt) dated 18 March 2026 superseded the 6 March 2026 notice. Online periods must be sufficiently compensated by physical onsite training at the parent foreign institution. If that is documented (compensatory certificate + transcripts) and verified, NMC’s FAQ line is that extra clerkship in India is not ordinarily required solely to duplicate training already completed abroad. If it was not compensated, the older 7 December 2023 clerkship path (one or two years in India) still applies. State Medical Councils verify the file.

UGMEB’s 18 March 2026 public notice (U-15024/15/2024-UGMEB) is a clarification of the 7 December 2023 and 19 June 2024 notices on FMGs who studied online because of COVID-19 or war, and on physical onsite compensatory classes. It withdraws the 6 March 2026 notice. A later UGMEB FAQ (as reported) states that FMGs who completed compensatory physical training and internship at the parent foreign institution, and can substantiate it, shall not ordinarily be required to undergo additional clerkship in India solely to duplicate that training.

Counsellor insight 2: For a 2026 joiner, the March notice is a warning: do not buy online/hybrid MBBS. For a pipeline FMG with COVID/war online years, print the 18 March 2026 PDF (not a 6 March screenshot). Any other “new NMC rule” on WhatsApp is usually FMGL 2021 restated, or a named-campus notice — if there is no NMC PDF, you do not have a rule.

Despite earlier alerts, NMC stated that Indian students were still taking admission in foreign private colleges whose curriculum, timeframe and training were not in consonance with NMC regulations, and again advised that FMGL conditions on duration, medium of instruction, syllabus and internship/clerkship are mandatory for registration to practise allopathy in India — with sole responsibility on the candidate in case of disqualification.

What happens if a university does not comply — and how should Indian families verify?
If the institution’s course does not meet current FMGL conditions, NMC’s advisory says graduates may be ineligible for FMGE. The Commission has said the onus lies solely with the student. There is no NMC refund window. Verification is before the first instalment: legal name, months, internship hospital, English on the wards, local licence, and a search of NMC notices for that exact entity.
This is the India-family hour. Education loans fund an offer letter. They do not audit FMGL. MBBS abroad fees is the six-year worksheet. Neither document replaces the circular.
In Green Park counselling the files that later break usually failed one of three checks: the internship hospital was a different legal name from the university; English was promised for theory and quietly dropped on the wards; or the family treated “NMC approved” as a list they had already seen on a consultant’s PDF. The families who stay solvent print the university circular, search nmc.org.in for that exact name, and keep the NEET scorecard inside the three-year window.
- Confirm a valid NEET result inside NMC’s three-year window from declaration (eligibility cluster).
- Write down the university’s full registered name — not a brand nickname.
- Collect the circular: duration in months, language including clinical years, internship hospital of the same institution, local graduate licence rule, fee years.
- Search NMC public notices and advisories for that exact campus (branch and offshore arrangements are where files fail). Uzbekistan needs a fresh named-campus check after April 2026 reporting.
- Confirm the degree confers a licence to practise for local graduates in that country.
- Pay only to the university account on the offer letter. Then follow the destination’s official visa/residence checklist — not a WhatsApp PDF.
Counsellor insight 3: Country is a living arrangement. Georgia’s campus detail is on MBBS in Georgia. Russia, Kazakhstan, Kyrgyzstan and Uzbekistan sit on the hub until dedicated slugs are live. FMGE rules follow the circular — months, internship letterhead, English in writing, local licence — not the flag.
NMC tells families to confirm fees, content, duration and internship with the university. That is why this office will not sell “50+ NMC universities.”
